2.1.5.1. Patient’s personal details (name, surname, email address, date of birth, gender, citizenship);

2.3. You, as the client, warrant that you have obtained the required consent from your patient, in terms of the National Health Act 61 of 2003, to upload his / her personal information on FindMyImplant.

3. Using personal information

3.1. The personal information, yours or the patient’s, submitted to us through our website will be used for the purposes specified in this policy, or in terms of the term and conditions of the website.

3.2. As such, we may use your personal information to:

3.2.1. administer our website and the business of FindMyImplant;

3.2.2. personalise our website for you browsing convenience;

3.2.3. enable your use of the FindMyImplant services available on our website;

4.1. FindMyImplant may disclose your or your patient’s personal information only to those of its employees or officers who are participating in the Services under this agreement and who need to know such information.

4.2. We may disclose your and your patient’s personal information under the following circumstances:

4.2.1. to the extent that we are required to do so by law;

4.2.2. in connection with any ongoing or prospective legal proceedings;

4.2.3. in order to establish, exercise or defend and legal rights FindMyImplant may have;

You expressly agree to the transfer of your and your patient’s personal information described in this section.

6.1. In respect of your and your patient’s personal information, we will retain those documents containing personal data:

6.1.1. to the extent that we are required to do so by law;

6.1.2. if we believe that the documents may be relevant to any ongoing or prospective legal proceedings; and

6.1.3. in order to establish, exercise or defend our legal rights FindMyImplant may have.
